Client's daughter serves as the primary caregiver and is also his paid caregiver through the agency, providing the majority of his day-to-day support.

Client has a diagnosis of dementia along with multiple chronic health conditions, including diabetes, hypertension, and high cholesterol. His cognitive impairment impacts memory, decision-making, and overall safety awareness. He also has limited mobility and uses a cane for ambulation, placing him at increased risk for falls. Due to his diabetes, consistent daily monitoring of blood sugar levels is required. He requires ongoing supervision and support to maintain his health and safety in the home.

Care should be provided in a manner that supports Client's comfort, safety, and independence, with consideration given to his cognition and mobility. Safety should remain a priority at all times, particularly with mobility and environmental awareness. Client's family plays a central role in his care, and coordination between the caregiver and agency is important to ensure consistency and continuity.

Maintain ongoing awareness of the client's condition and baseline. Monitor for any changes—illness, infection, urinary symptoms, bowel or bladder patterns, constipation, weight changes, intake (food/fluid), behavior changes, cognitive shifts, confusion, or any emerging concerns.

Any changes or significant events—such as incidents, falls, or the need for emergency care—must be addressed in real time. This includes taking appropriate action at the time of care to ensure safety and escalation as necessary, contacting emergency services when needed, and seeking medical guidance as appropriate.
