Susan is a pleasant and cooperative woman who lives with her husband in their two-story family home. Caregivers provide companionship, supervision, and assistance with daily routines, including bathing, dressing, and maintaining personal hygiene. Caregivers may also accompany Susan and her husband on shopping trips, errands, and appointments as needed.
Susan has dementia and reduced safety awareness and requires continuous supervision. She must remain within the caregiver's sight at all times and should not be left unattended while care is being provided. Exterior doors in the home do not currently have specialized safety locks, so caregivers should remain attentive to door access while maintaining visual supervision.
The basement door off the kitchen presents a fall risk. While Susan does not typically approach the basement door, caregivers should remain attentive to her whereabouts and keep the door closed. Gently redirect Susan away from the basement door if needed. The family plans to install a lock for added safety.
Susan may be resistant to bathing. Hygiene should be approached calmly with encouragement rather than pressure, accepting the level of care she is willing to tolerate during the visit. Some days this may include a full shower, while on other days assistance with changing clothing or completing basic hygiene may be more appropriate. Her husband monitors overall hygiene needs and will advise when a more complete bath or shower should be attempted.
Susan moves about the home independently, including using stairs. Hands-on assistance is required when navigating outdoor stairs and curbs.
A calm, confident, and reassuring approach works best. Susan generally responds well to gentle direction and tends to follow the caregiver's lead when approached in a supportive and respectful manner.
Assist with meal preparation as needed. Ensure meals are prepared according to the client's preferences and dietary needs.
Assist with hygiene needs including grooming, oral hygiene, and maintaining personal appearance. May include basic hygiene care or changing clothing depending on client's willingness.
Full assistance with personal care routines as needed. Coordinate with family regarding overall hygiene needs and care preferences.
Full assistance with bathing. Approach calmly with encouragement. Accept the level of care the client is willing to tolerate that day.
Assist with upper body dressing. Provide hands-on help as needed while encouraging independence.
Assist with lower body dressing. Provide hands-on help as needed while encouraging independence.
Monitor walking and movement. Hands-on assistance required for outdoor stairs and curbs. Susan is independent indoors.
Monitor during meals. Provide assistance as needed based on client's needs during that visit.
Susan must remain within caregiver's sight at ALL times. Continuous monitoring required due to dementia and reduced safety awareness. Do not leave unattended while care is being provided. Remain attentive to door access.
