Detailed Care Instructions

102 - Bath-Bed

Provide bed bath assistance as needed when Joe requests or when shower is not feasible. Ensure privacy and maintain dignity during all personal care activities. Use warm water and appropriate cleansing supplies. Check skin for any redness or pressure areas during bathing.

106 - Hair Care-Shampoo

Assist with hair washing and shampooing during shower or bath as needed. Joe may need standby assistance due to balance issues when leaning over the sink. Ensure hair is thoroughly rinsed and dried. Brush/comb hair neatly.

115 - Meal Preparation

Assist with meal preparation and light meal planning due to Joe's balance challenges. Prepare breakfast, lunch, and dinner as requested. Ensure kitchen is clean after meal preparation. Offer healthy options and encourage adequate nutrition and hydration. Joe should remain seated while food is being prepared to prevent falls.

116 - Housework/Chores

Assist Joe with keeping his apartment clean and orderly. IMPORTANT: Do NOT rearrange, remove, or discard any items without Joe's knowledge and agreement - his home contains meaningful artifacts from his anthropology career. Keep all walkways clear of tripping hazards. Complete light housekeeping tasks including dishes, tidying, vacuuming as needed.

117 - Managing Finances

Provide practical assistance with finances including organizing bills, writing checks at Joe's direction, and preparing payments for mailing. NEVER use Joe's debit card, withdraw cash, or access his accounts without his explicit permission and direction. Document any financial assistance provided.

118 - Managing Medications

Joe manages his own medications but benefits from daily reminders and confirmation that he has taken them. Provide gentle reminders at regular intervals. Confirm he has taken his medications by asking and documenting. Do not physically handle his medications unless specifically asked.

119 - Shopping

Assist with grocery shopping and errands as requested. May need to accompany Joe to stores or shop on his behalf. Maintain the shopping list and update as needed. Ensure proper handling of perishables upon return.

120 - Transportation

Provide transportation as requested. Hands-on assistance is required for stairs, curbs, and uneven surfaces. Ensure Joe is properly secured with seatbelt at all times while vehicle is in motion. Assist with entering and exiting the vehicle safely. Never leave Joe alone in the vehicle.

122 - Hygiene

Assist with hygiene care daily in the morning and as needed throughout the day. Provide standby assistance and hands-on support as needed. This includes assistance with washing face, hands, and personal hygiene tasks. Allow adequate time - these routines can take 2+ hours due to Joe's balance challenges.

123 - Dressing Upper

Assist with dressing upper body daily in the morning and evening. Provide standby assistance and hands-on support as needed. Allow adequate time for Joe to dress himself where possible while ensuring safety. Assist with buttons, zippers, and clothing adjustments as needed.

124 - Dressing Lower

Assist with dressing lower body daily in the morning and evening. Provide standby assistance and hands-on support with putting on/removing pants, socks, and shoes. Ensure proper footwear for safety. Allow adequate time for these tasks.

125 - Locomotion

Joe ambulates independently but can be unsteady. Provide standby assistance during all ambulation - be ready to provide hands-on support if he loses balance. Clear pathways before walking. Use proper body mechanics when providing support. Joe is at risk for falls due to unsteady gait.

126 - Transfer

Provide standby assistance during all transfers: bed, toilet, shower, and wheelchair. Be ready to provide hands-on assistance as Joe can be unsteady. Use proper transfer techniques to ensure safety. Maintain physical proximity during all transfers.

127 - Toilet Use

Assist with toileting as needed. Provide standby assistance and help with clothing adjustments. Ensure toilet is clean after use. Monitor for any signs of urinary or bowel issues. Provide privacy while maintaining safety.

128 - Bed Mobility

Provide standby assistance with repositioning in bed as needed. Assist with getting in and out of bed using proper transfer techniques. Ensure bed position is comfortable and safe. Check skin for redness during repositioning.

129 - Eating

Report any concerns (constipation, diarrhea, blood, unusual odor) to the office immediately. Provide prompt assistance with cleaning and hygiene. Maintain dignity and privacy during care.

132 - Personal Care T1019

Provide hands-on assistance with personal care activities including washing face, brushing teeth, combing hair, and grooming. Assist with oral care. Provide nail care for fingers as needed. Ensure Joe looks neat and well-groomed at all times. Maintain dignity and privacy during all personal care activities. Allow adequate time - these routines can take 2+ hours.

134 - Bathing - Wash Up

Assist with washing up at the sink and during shower/bath as needed. Provide standby assistance and hands-on support as required. Due to balance challenges, Joe may need support when standing at the sink. Ensure proper water temperature and safety. Maintain privacy and dignity.

137 - Lotion/Ointment

Apply lotion or ointment as needed or requested. Pay attention to dry skin areas. Apply when Joe is seated safely - not standing. Note: Lotion can make skin slippery on feet. Ensure Joe wears non-slip footwear or socks after application. Inform Joe that feet may be slippery after application.

138 - Laundry

Assist with laundry as needed. This may include washing, drying, folding, and putting away clothes. Ensure laundry is completed in a timely manner. Check that Joe has clean clothing available. Do not discard or alter any of Joe's belongings without his explicit permission.

139 - Reading/Writing

Assist with reading mail, documents, or other materials as needed. Help with writing notes, letters, or forms as requested. Provide reading assistance for any important documents. Maintain confidentiality of any personal information.

140 - Supervision/Coaching/Cueing

Provide standby supervision during all care tasks. Coach and cue Joe as needed to promote independence while ensuring safety. Provide verbal cues and guidance for tasks he can do with some assistance. Encourage Joe to remain active and engaged while being mindful of his safety due to his unsteady gait.

141 - Incontinence Care

Provide prompt assistance with incontinence care as needed. This includes checking, changing, and proper disposal of incontinence products. Maintain proper hygiene and skin care. Report any skin irritation or concerns to the office. Ensure Joe remains clean and dry. Maintain dignity and privacy at all times.
