Care Narrative

Patty is an alert and oriented senior woman living alone in a second-floor apartment with her cat.

She manages many aspects of her daily life independently but requires support to stay on track with meals, medications, and day-to-day responsibilities. Support with meal planning and execution is important for blood sugar management. She also benefits from companionship and regular interaction.

Patty ambulates independently but is at increased risk for falls. She is generally independent with personal care but benefits from standby assistance for safety. Hands-on support is needed when navigating stairs or uneven surfaces.

Her care is supported by a nurse case manager through St. Mary's and an assigned social worker who assist in overseeing services.

Communication should remain calm, straightforward, and supportive. Information should be shared carefully, with attention to accuracy and relevance to care. Questions or concerns are directed to the office for coordination with the nurse case manager and social worker.
