Person Centered Service Plan
Effective: 2-26-2026
Ron is an alert, independent, and sociable gentleman living alone in senior housing. He has mobility limitations but is able to ambulate independently within his apartment using a walker. For longer distances and outings, he uses a scooter for safety and convenience.
Ron requires assistance with instrumental activities of daily living (IADLs), including light housekeeping, laundry, meal preparation support, and running errands. For personal care tasks like hygiene and dressing, caregivers should offer assistance and be available to provide standby or hands-on support as needed, but allow Ron to direct his own care whenever possible.
Ron enjoys light conversation and companionship during visits. Caregivers should be friendly and personable while respecting his independence. He is flexible with scheduling and prefers visits after 10:00 AM. The typical schedule is 2-3 hours per visit, 3-4 days per week, depending on his needs and caregiver availability.
