Pennsylvania law requires disclosure of any criminal history as part of the hiring process for home care workers.

Why We Ask

Pennsylvania regulations require home care agencies to conduct criminal background checks on all employees who provide direct care services. You must honestly disclose any criminal convictions or pending charges during the hiring process.

How We Evaluate

Having a criminal record does not automatically disqualify you from employment. Silver Home Care evaluates each situation individually, considering:

  • The nature and severity of the offense(s)
  • How much time has elapsed since the offense
  • Whether the offense is relevant to the position
  • Your references and work history
  • Evidence of rehabilitation

Critical Requirement

Providing false, misleading, or incomplete information will result in immediate termination upon discovery, regardless of when the information is revealed.

Confidentiality

All information you provide will be kept confidential and used only for employment eligibility determinations in accordance with state and federal law.

Criminal Record Disclosure Form

Have you ever been convicted of, pleaded guilty or no contest to, or are you currently facing charges for any criminal offense (excluding minor traffic violations)?

For each offense, please include: (1) the nature of the offense, (2) the date of conviction or charge, (3) the location (city/state), and (4) the outcome/sentence.

This form will be submitted securely to Silver Home Care HR. You will receive a confirmation email once submitted.
